Our Story
Evergreen Adult Medicine opened in Thurmont, Maryland, in 2005. The practice was co-founded by Talbot Smith, MD, and it is still family-owned today. Dr. Smith leads the clinical team and sees patients himself, and several of our staff members have been with us for 9 to 20 years.
For more than 20 years, we’ve provided primary care, hormone therapy, and testosterone treatment to patients in Frederick County and the surrounding area. Evergreen TRT brings that care into a fully virtual program: appointments are online, and no office visit is required. Once prescribed, medication is shipped straight to your home. You work with the same providers our local patients have seen for two decades.

Dr. Smith co-founded Evergreen Adult Medicine in 2005 and has led its clinical team ever since. He is board certified in family medicine and continues to see patients directly.

Do I need to come into the office?
No. Evergreen TRT appointments are fully virtual, and medication is shipped directly to you when prescribed. Your provider will explain any lab work needed and how to complete it.
How do I know if TRT is right for me?
Every patient has a medical review with one of our licensed providers before any treatment decision is made. If TRT isn’t the right fit, your provider will talk through other options with you.
Is there a minimum commitment?
Yes. Evergreen TRT is structured as a 6-month program, which your provider will walk you through before you enroll.
